A current major concern of using DES is very late stent thrombosis (VLST) that may occur beyond 1 year after DES implantation (1). VLST is an infrequent, albeit catastrophic complication, which can lead to myocardial infarction or sudden cardiac death (2). However, the underlying mechanisms of VLST remain unclear. In the current report, we show 3 cases of VLST after sirolimus-eluting stent (SES) implantation. Findings from coronary angioscopy (CAS) and optical coherence tomography (OCT) suggested 3 differential mechanisms of VLST in each case.

Mechanisms of late stent-related myocardial infarction: insights from optical coherence tomography
This article focuses on optical coherence tomography findings in patients with late major cardiovascular events like myocardial infarction and/or stent thrombosis, following drug-eluting stent and bare-metal stent implantation. Uncovered stent struts and malapposition seem to play a critical role in drug-eluting stent-related late events. Neointimal features similar to native atherosclerosis ar...
متن کاملClinical use of optical coherence tomography to identify angiographic silent stent thrombosis
OBJECTIVES Patients previously treated with coronary stents may suffer an acute coronary syndrome (ACS) without any evidence of thrombus formation on coronary angiography (CAG). This may be due to partial, nonocclusive stent thrombosis with microembolization. In this paper, we illustrate possible mechanisms both with optical coherence tomography (OCT) and histology. DESIGN We present two case...
متن کاملLate stent malapposition combined by thrombus resolution after primary stenting in acute myocardial infarction: optical coherence tomography findings.
The late stent malapposition (LSM) is defined as a separation of the stent struts from the intima of the coronary artery wall which was not present just after implanting the stent. Although drug-eluting stents (DESs) dramatically reduce the in-stent restenosis, several studies have reported a higher frequency of LSM on follow-up intravascular ultrasound (IVUS) examination after DES implantation...
متن کاملCharacteristics of Earlier Versus Delayed Presentation of Very Late Drug‐Eluting Stent Thrombosis: An Optical Coherence Tomographic Study
BACKGROUND The pathophysiology underlying very late drug-eluting stent (DES) thrombosis is not sufficiently understood. Using optical coherence tomography, we investigated characteristics of very late stent thrombosis (VLST) according to different onset times. METHODS AND RESULTS A total of 98 patients from 10 South Korean hospitals who underwent optical coherence tomography for evaluation of...
متن کاملStent Evaluation with Optical Coherence Tomography
Optical coherence tomography (OCT) has been recently applied to investigate coronary artery disease in interventional cardiology. Compared to intravascular ultrasound, OCT is able to visualize various vascular structures more clearly with higher resolution. Several validation studies have shown that OCT is more accurate in evaluating neointimal tissue after coronary stent implantation than intr...
